If you wish to use the form below, you may request a medication you have been prescribed before. Please note that this may need to be authorized by your own GP and that this may take up to 5 days, as doctors work on different days.
Please allow at least 72 hours (3 working days), before your Prescription will be ready for collection.
Please note: For reasons of privacy this form will not store your details or medication request. There is no email acknowledgement with this service. Once you send this form a notification message will appear to indicate successful submission.
